[V6-12v] Hesitation

Hi All --

I'm looking for ideas on a hesitation problem that I'm still having on the 
car. It is intermittent, which makes it hard to track down (especially since it 
doesn't seem to be leaving any fault codes), but it will happen at least a 
couple of times on each journey. Symptoms are as follows:

1. When pushing the throttle down slightly from whatever position it has been 
previously, the car will hesitate (stay at the same revs) for between half a 
second to two seconds. Pushing your foot down even further will usually make 
no difference, the revs will go up when they feel like it. Funnily though, if 
you back off the throttle at this point, then put your foot down again, the 
problem usually disappears.
2. This tends to happen at two distinct moments. Firstly, when starting from 
cold, the car will generally do this the first time it encounters some 
resistance (e.g. a slight incline), or the first time you press the throttle more 
than lightly. This goes once the car has warmed up. However, it also occurs when 
the car is up to temp if you have been waiting at a junction with the engine 
at idle, or if you've been cruising at low revs -- the first time you put your 
foot down after that it'll hesitate sometimes. It's particularly annoying at 
these times, in fact, because you generally WANT reliable power when you're 
pulling out of a junction!

The car is otherwise in perfect running order, and shows no other performance 
problems.

Any ideas?
