[V8] Bill of Sale / Purchase Agreement

I just started the process of adding the 4th Audi to the Fleet.
 
I'm in need of a Bill of Sale / Purchase Agreement for the deal.
 
Can someone send me a copy of form that they have used in the past.  If you have a state specific contract the transaction will take place in Washington state.
