Impossible? No.  Inconvenient, maybe.  

Treat it as you would if you had to move to a new location due 
to growth.  Make the announcement in advance (I'm switching
from AOL to newISP), let them know a) at the start of the switch 
(I can new be reached at me at newISP) and at the end (my AOL 
account will no longer be active).

Give them a 1 to 3 month window of overlap. Do it during the
"slow" season (if you have one), to give them time to adjust.
Make your announcements via email (easier for them to update)
and, for critical accounts, via a postal mailing.

Some sales people find any excuse to talk to a customer is
a good one.  "Hey, we haven't talked in a while. I just wanted
you to know that we are switching ISPs. To better serve you,
we can be reached now at x at new.com. You can also check 
out our new, faster web site at www.y.com."
