Seriously??! "How do you fail to tighten a 26MM Nut"?? You need to take
a very good look at the upper strut mount AND its mounting points to
look for damage there or you'll never get the car aligned correctly
again.

That washer is on there not only to provide tension to the bushing but
also as a safety should the bushing fail - the control arm cannot go
past it. Coupled with a 26MM nylock nut - unless it's stripped how do
you get all that stuff to "just fall off" neatly where you can find it
again - Yikes!. 

Good Luck getting her back together and straightened out.
