[V8] the clutch did fail. #$&#%#

it looks like my pressure plate failed.  Here is a link to some pictures:

this is on my laptop over a pretty slow DSL connection so it may or may
not work for you but there is a picture or two that clearly shows the
unevenness of the fingers.  There is also some weird color banding on both
the flywheel and pressure plate, along with some blue spots.  Does this
look normal?  has this thing been slipping?  Thinking back, this has
probably been bad almost since I put it in.  I have always had trouble
getting into reverse and second, but I just thought the guy who rebuilt
the transmission did a crappy job.

Now what to do.  It seems all the available pressure plates are Sachs. 
Did I just get a dud?
