I received a 4 speed pressure plate when I needed a 5 speed plate once.  I 
didn't notice the wrong piece. So I installed it.  I discovered that the 
clutch didn't fully engage when I was done it slipped just like mad and no 
amount of loosening the cable would fix it.
The 5 speed mainshaft extends slightly further into the center of the clutch 
assembly than the 4 speed.  So the small push disc that clips into the 
center of the pressure plate for a 5 speed is dished further in toward the 
crank.  The difference doesn't jump out at you unless you know it is there.
If this is what is wrong you'll have to pull the tranny again and put in the 
correct pressure plate.  If you have some used pressure plates you could 
just swap in the 5 speed push disc as long as the spring clip cutouts match 
the one on the pressure plate.
