  If the inspection/timing hole has been open, anything could be 
in there.  I've seen the hardened, pivot spot in the middle of the 
pusher plate, come loose.  
  5th gear problems are almost always from low lube.  Usually full 
to the hole but most of the "earlier" 5 speeds have the level check 
hole at the wrong level.  Drain and fill with 2 quarts (or even 
measure and refill.)
  5th gear is accessable by removing the end cover.  You may 
be able to adjust the setting on the shift fork a little but I'd see 
what the lube does first.  I'd drain and fill with synthetic.
