Dave, to reiterate what others have said, your insurance company should be
working as an intermediary between you and the other drivers' insurance.
 In my experience (and this was with a non-total, although I don't see why
it would make any difference), *your* insurance would cover the expenses to
"make you whole", and then they would recoup their costs from the other
guys' insurance and duke it out with them if there were a disagreement.

Have you been keeping your agent in the loop about this?  What do they have
to say?

Have you seen a doctor about your neck pain yet? :)  One thing they will
expect is for you to surrender any rights to future compensation for
injury/pain/suffering/etc before they pay you, and they should certainly be
willing to pay for you to surrender those rights.
