Standard hex key (17mm I think)

I filled mine fairly slowly (half an hour or so) with the car tilted.  I 
then drove it for a week and topped it up with the car level.  It took about 
another half pint.

I didn't measure it exactly (I spilled a bit until I worked out a 
spill-proof way of doing it), but the Bentley manual is normally fairly 
accurate.

Can't remember.  Don't confuse it with the 5th gear switch used for the 
change up indicator.

I had a LOT of wear in mine - so much that when the car got hot, I couldn't 
engage reverse or fifth.  It turned out to be all due to the rivet that 
holds the stainless steel ball onto the link arm from the transmission.  I 
bought another of these arms with ball attached for about GBP30 and 
everything worked fine (the replacement only took a few minutes - the 
hardest part was popping the ball and socket and reinserting afterwards).
