IME it's _very_ unusual for a CV joint to fail inside an intact boot.  The damn 
things seem to be bulletproof as long as their nice clean, lubricated 
environment is maintained.  Rip a boot - and you start counting down to 
problems.

I go for the sticking guide bolts now, with a side bet on a loose steering rack 
mounting bolt.
