90Q20V Cold Start Problem

My 1990 90Q20V stalls after the firt the cold start in the morning. Car
runs for a minute then the RPM drop and it stalls. Second start the car
runs then RPM drop, a little gas, all is well. Start to drive and the
car bucks a bit the, all is well. By the time I get to the first stop
sign 1/2 mile away, the RPM drops to about 500 the up again.
I cleaned the ISV, still have the same trouble. I run Sunoco 94, and use
the Audi gas additive every 4 tanks of fuel. Ideas? A new ISV is abour
300$CDN. Car has 238,000km.
