'89 200TQ Start/Idle Woes...

The first time I turn the key to start the car, it turns over but
doesn't start.  The second time, it always starts.  

After running for a while (~30 mins), it tends to stall at idle.
The Tach says ~700 RPM, but I'm not sure I can trust it given that the
instrument cluster needs to be properly grounded.  When the car
stalls, I have to let it sit for a few mins or so and then it starts
up.

Are these two symptoms related?  What could this be beside the ISV?
