Northern English dialect.  "Something needs adjusting".

I think you probably know the expression: "In fine fettle".  Fettle is
actually a verb, and you occasionally still see job ads for 'fettlers'
in the North - they're people who tidy up metal castings by taking off
the rough edges and mould marks.
