Once it's a fair way out, you have to push like crazy and turn at the
same time until the threads engage.

Also - did you replace the seals?  A while back, several listers (including
me) got caliper kits that had slightly undersized piston seals in them.

The piston wouldn't go in unless it was forced, and then it bit a chunk
out of the seal.
