Mis-behaving 4kq: FINAL

Finally got around to replacing the injector on #2.  Voila.  No more
cold-start stumble and no more missing cylinder.  All is well in Q-land,
finally.

Thanks to all.. esp. to Phil, who diagnosed my cold-start-5-second-stumble
about 3 months ago.. I still have that post/reply--spot on!
