It sounds like the fan may be on its last legs there.  New ones are 
under $100, but a bit of a pain to replace (have to pull the dash back a 
bit to get them in/out).

If it is having trouble spinning, the lower voltage it sees on settings 
1-2-3 may not be enough to get it to run properly.
