Adrian,
Those symptoms mirror the experience I had with a spark plug that worked it's way loose - eventually requiring me to retap the sparkplug hole. The vibration was severe enough to practically disintegrate the spark plug. As a caution I pulled the head clean up the mess.
For me it was Cylinder #5. Now I check the torque on the spark plugs with each oil change. You'll know right away if that's the problem.
