[V8] Strange issue

A friend of ours is having a strange issue with his V8; he has asked for my
help, but I am stumped...  Anyone have any ideas?

Problem:
Only happens when cold.  Every morning, he starts the car, waits 5min with
it in Park, and the engine sounds fine.  When he starts to drive it, the
first 10 miles are pretty rough - the engine chokes, shakes, and he has no
power.  If he stops at a light with the transmission in drive, it has a very
rough idle.  After 10 miles, the car works just fine, has plenty of power,
and no vibrations.  If the warm engine sits and cools for more than a few
hours, it starts all over again...

Any ideas for Vladimir?  Obviously pulling the codes would be a start, but
the only issues I can think of (bad valve body in the trans, bad spark
plugs, wires, swapped plug wires) don't go away when the car warms up...  Or
do they?
