  The way I see it there are two possibilities.  The first and most commmon 
is 
that the cam timing was a bit retarded (I'd guess) so the piston nailed the 
valve but just enough to punch the lifter rather than bend the valve.  Still, 
I've 
never seen one loose any keepers when that's happened which brings me 
to the second possibility.  The keepers weren't seated properly, came loose 
and allowed the valve to "flop" about.  With the type of valve stem seals 
these have, it wouldn't drop clear in, the piston would push it back up and 
so on.  I tend to lean toward the second possibility.
  Stacked head gaskets on a diesel???  Big no, no IMO.  There should be 
no clearance issue between the head and block, unless the piston projection 
is way beyond 1.6 specs.  Still, a little advancing of the timing or shaving 
of the 
piston should and would (respectively) take care of that.  Otherwise the 
valve 
lift is the same as always and the piston rotates away almost the same, 
depending on difference in stroke.
  There's usually just a dusting of carbon on the pistons.  Seems like there 
might be a dab more to indicate where the valves are.  If it's much then I'd 
say your valve stem seals or guides are letting too much oil past.
