Brian,

I have been following your problem and I seemed to have missed
any mention of you hiring a lawyer.  You shouldn't really be wasting
your time and effort dealing with the insurance, you have to play
them at their own rules. 

If you don't want to hand over any money to lawyers, call Harvard
Law, they have third year law students who handle pro-bono
cases and if they cannot handle the case they will surely
refer you a lawyer who can.
