John,
Statistically, the most likely cause is that when the bearings were
fitted, they were pressed in while pushing on the inner race. This
causes the balls to indent the bearing surfaces and leads to early
failure. 2nd most likely cause is not replacing the CVJ nut/bolt with
new, that can cause the nut/bolt to come loose and results in excess
freeplay. 3rd most likely cause is not correctlt torquing the nut/bolt
also causing it to come loose.
