(5000tq/200) avant 3rd seat?

I am in the market for a 5000tq/200 Avant and would like to
understand the 3rd seat option a little better:

 1) Which model years had the 3rd seat available?

 2) How rare is this option?

 3) Is this available as a retrofit?

 4) Has anyone tried to fit seats from another kind of car,
    e.g., a Taurus.
