IME the normal mode of failure is a catastrophic water leak - with the
engine running, it pours out faster than you can pour it in.

Secondary failure is dragging the belt off the idler roller.  God in
His wisdom knows how, because the belt is fed to the idler by the
crank pulley which has guides and walls to keep the belt straight.
