The wiring could be lose, but I suspect the actual knob is probably at
fault... it either needs to be cleaned or repladed. (If you end up having to
remove the radio, your best bet would be to replace it with an aftermarket
radio).

It could be the switch... best way to check is to disconnect the switch and
put it on one of the windows that you know works and see if you get the same
behavior out of the switch. The next thing I would check if the switch isn't
at fault is the wiring going into the door... many times the boot becomes
weather checked and will slip out of place exposing the wires to getting cut
and/or pinched. If those are fine, I would check the motor... however I
had a regulator that acted similar to yours and just replaced it in the
end.... (I think it was shorting out when it hit the bottom... Couldn't get
it to go up until half an hour after I turned the car off)...
This is the way I check the windows... some other listers may do it
differently...
