bad heater core.  they can leak just a wisp for years (mine did).  they
can just go bust.  varies.  And yours is a total pita to change I
think!  (Marc Swanson & I "removed" the climate plenum on his donor car
today with a crowbar and big hammer.  i would not want to have to remove
one whole - and the heater core is embedded in the middle of it.

maybe.  but I bet it related to your hard starting, at least vaguely...

it would make sense to fix known broken items (like that exhaust leak,
and the nasty noise from underneath) first, since they certainly make it
harder to diagnose other things.
